Pelvic Ultrasound (Transabdominal)

What Is a Pelvic Ultrasound (Transabdominal)?

A Pelvic Ultrasound (Transabdominal) is a safe, noninvasive imaging examination that uses high-frequency sound waves to create detailed, real-time images of the pelvic organs through the lower abdomen. This examination allows evaluation of structures such as the uterus, endometrium (uterine lining), ovaries, cervix, and surrounding pelvic tissues without the use of radiation, needles, or contrast dye.

A transabdominal pelvic ultrasound is commonly performed to investigate pelvic pain, abnormal uterine bleeding, ovarian cysts, pelvic masses, uterine fibroids, and other conditions affecting the female reproductive system. It may also be used to evaluate the cause of pelvic fullness, monitor previously identified abnormalities, or follow up after treatment.

During the examination, a handheld ultrasound transducer is gently moved over the lower abdomen while your bladder is comfortably full. A full bladder helps improve visualization of the pelvic organs by providing a clear acoustic window for imaging.

A Pelvic Ultrasound (Transabdominal) is a painless examination that typically takes 20–30 minutes to complete. It provides valuable diagnostic information while avoiding exposure to radiation, making it an excellent first-line imaging test for evaluating a wide variety of pelvic conditions.

Why Is a Pelvic Ultrasound (Transabdominal) Performed?

A Pelvic Ultrasound (Transabdominal) is performed to evaluate the female pelvic organs and help identify the cause of pelvic symptoms or abnormal findings. It provides detailed, real-time images of the uterus, ovaries, cervix, and surrounding pelvic structures without the use of radiation.

This examination is commonly performed to investigate pelvic pain, abnormal uterine bleeding, ovarian cysts, uterine fibroids, pelvic masses, or unexplained pelvic fullness. It can also help evaluate enlarged ovaries, ovarian torsion, fluid within the pelvis, and other conditions affecting the female reproductive organs.

A Pelvic Ultrasound (Transabdominal) is frequently used to monitor previously diagnosed pelvic conditions, follow up after treatment, or evaluate abnormalities identified during a physical examination or on other imaging studies. It may also be performed to assess pelvic anatomy in patients who are not candidates for a transvaginal ultrasound or when a broader view of the pelvis is desired.

Because ultrasound provides excellent visualization of the pelvic organs while avoiding radiation, contrast dye, and invasive procedures, it is often one of the first imaging tests recommended when a pelvic condition is suspected. The results help guide diagnosis, treatment, and any additional evaluation that may be needed.

Common Reasons to Schedule a Pelvic Ultrasound (Transabdominal)

A Pelvic Ultrasound (Transabdominal) is performed to evaluate the female pelvic organs and help identify the cause of pelvic symptoms, abnormal physical examination findings, or previously diagnosed pelvic conditions. You may benefit from this examination if you are experiencing pelvic discomfort or require evaluation or follow-up of a known gynecologic condition.

  • Pelvic pain or pressure

  • Abnormal uterine bleeding

  • Heavy or prolonged menstrual periods

  • Irregular menstrual cycles

  • Ovarian cysts

  • Uterine fibroids

  • Pelvic masses

  • Enlarged ovaries

  • Suspected ovarian torsion

  • Pelvic fullness or bloating

  • Fluid within the pelvis

  • Follow-up of previously diagnosed pelvic conditions

  • Monitoring ovarian cysts or uterine fibroids

  • Evaluation of abnormal findings from another imaging study or physical examination

A Pelvic Ultrasound (Transabdominal) provides a comprehensive evaluation of the pelvic organs, helping identify abnormalities involving the uterus, ovaries, and surrounding pelvic structures. It is a safe, painless, and radiation-free examination that provides valuable information to guide diagnosis, treatment, and follow-up care.

What to Expect During Your Examination

A Pelvic Ultrasound (Transabdominal) is a safe, painless, and noninvasive examination that typically takes 20–30 minutes to complete. During the study, you will lie comfortably on an examination table while an experienced ultrasound technologist performs the examination.

You will be asked to arrive with a comfortably full bladder, as this helps improve visualization of the pelvic organs. A warm, water-based ultrasound gel will be applied to your lower abdomen, and the technologist will gently move a handheld transducer across the area to obtain detailed images of your uterus, ovaries, cervix, and surrounding pelvic structures.

During the examination, you may be asked to change your position slightly or briefly hold your breath while certain images are obtained. These simple instructions help provide the clearest possible images.

The examination is generally very comfortable. You may feel mild pressure from the transducer, especially if your bladder is full or if the area being examined is tender, but the procedure should not be painful. There are no needles, injections, contrast dye, or radiation involved.

Once all of the necessary images have been obtained, the gel will be wiped away, and your examination is complete. Your interpreting physician will review the images, and a report will be sent to your healthcare provider. If you scheduled your examination as a self-pay patient, we will explain how and when your results will be communicated.

How to Prepare for Your Pelvic Ultrasound (Transabdominal)

Proper preparation helps ensure the clearest possible images of your pelvic organs and allows your examination to be completed accurately and efficiently. Arriving with a comfortably full bladder is an important part of this study.

Before Your Appointment

✓ Drink 24–32 ounces (3–4 cups) of water about one hour before your appointment.

✓ Do not empty your bladder before the examination. A comfortably full bladder helps improve visualization of the uterus, ovaries, and other pelvic structures.

✓ You may eat and drink normally unless your healthcare provider has given you different instructions.

✓ Continue taking your usual medications unless instructed otherwise.

✓ Wear comfortable, loose-fitting clothing that allows easy access to your lower abdomen.

✓ Bring any previous pelvic imaging studies or reports if they were performed at another facility.

✓ Arrive 10–15 minutes early to complete any necessary paperwork before your examination.

Following these preparation instructions helps ensure the best possible visualization of your pelvic organs, allowing your Pelvic Ultrasound (Transabdominal) to be performed as accurately and efficiently as possible.

Frequently Asked Questions

Q: Does a transabdominal pelvic ultrasound provide as much detail as a transvaginal ultrasound?
A: A transabdominal pelvic ultrasound does not typically provide the same level of detail as a transvaginal ultrasound because the images are obtained through the lower abdomen rather than from within the pelvis. However, it still provides excellent information about the uterus, ovaries, bladder, and surrounding pelvic structures and is often the preferred first imaging examination. It is also the appropriate option for patients who cannot or prefer not to undergo a transvaginal examination.

Q: Do I need a full bladder for the examination?
A: Yes. A comfortably full bladder helps improve visualization of the pelvic organs by creating a better acoustic window for the ultrasound. Please follow the preparation instructions provided before your appointment.

Q: Is a pelvic ultrasound painful?
A: No. The examination is generally painless. You may feel mild pressure from the ultrasound transducer or discomfort from having a full bladder, but the procedure should not be painful.

Q: Is there any radiation involved?
A: No. Pelvic ultrasound uses sound waves rather than radiation, making it a safe imaging examination.

Q: How long does the examination take?
A: Most transabdominal pelvic ultrasound examinations take approximately 20–30 minutes, depending on the reason for the study and the images that need to be obtained.

Q: Do I need a referral to schedule a pelvic ultrasound?
A: No. You can schedule a Pelvic Ultrasound (Transabdominal) with a healthcare provider's referral or as a self-pay patient. Your examination will be performed by an experienced ultrasound technologist, interpreted by a physician, and our team is always happy to answer any questions before or after your appointment.
